During the past 40 years there have been major conceptual and technical advances in the domain of dialysis methods and renal/organ transplantation for long-term treatment of patients with End-Stage Renal Failure (ESRF). This now enables better defined, more selective strategies of treatment to be undertaken, according to patient-specific criteria such as age or underlying renal disease also taking into account the lifetime duration of these treatments. For many patients this implies necessary successive changes of mode of therapy depending on their availability, occurrence of medical/technical complications or failure and social environment and economic factors. This rationale has inspired the structure of this volume which is divided into four sections: 1/A descriptive overview of the various modes of renal replacement therapy (RRT): Extracorporeal dialysis/filtration, peritoneal dialysis, kidney and multi-organ transplantation. 2/Most appropriate indications and use of these methods, respective advantages, drawbacks and outcome in children, pregnant women and elderly patients. 3/ In diabetic patients and in patients with heredity/congenital diseases.4/ The ethical issues generated by this new domain in Medicine by limitations in treatment facilities or medical dilemas for acceptance, best technical choices, withdrawal or termination of RRT in individual patients. Finally, contributors from Eastern European Coutnries, Africa and Far Eastern Countries analyse the current status of RRT in their respective geographical area and the ways and means required for a wider implementation of RRT in thus far lesser economically developed countries where the great majority of the populations still have no access to these life-saving therapeutic procedures.

Meredith Wallace, PhD, APRN-BC, has been a nurse since she completed her BSN nursing degree Magna Cum Laude at Boston University in 1988. Following this, she earned an MSN in medical-surgical nursing with a specialty in geriatrics from Yale University and a Ph.D. in nursing research and theory development at New York University. During her time at NYU she was awarded a predoctoral fellowship at the Hartford Institute for Geriatric Nursing. In this capacity she became the original author and editor of Try This: Best Practices in Geriatric Nursing series. In 2001, she won the Springer Publishing Company Award for Applied Nursing Research. She was the Managing Editor of the Journal of Applied Nursing Research and is currently the research brief editor for the journal. She is the author of numerous journal articles and book chapters. Dr. Wallace published Prostate Cancer: Nursing Assessment Management and Care, (Springer Publishing Company, 2002), which won the American Journal of Nursing Book of the Year Award. Preceding this, she was the Associate Editor of The Geriatric Nursing Research Digest, which also won the AJN Book of the Year Award in 2002. She is the recent recipient of the Eastern Nursing Research Society/John A. Hartford Foundation junior investigator award. Dr. Wallace currently an Associate Professor and the Elizabeth DeCamp McInerney Chair in Health Sciences at Fairfield University School of Nursing.

Lorrie L. Powel, PhD, RN, is currently a postdoctoral fellow at the Boston University School of Public Health and the Center for Health Quality, Outcomes and Economic Research at the Edith Nourse Rogers Memorial Veterans Hospital, Bedford, Massachusetts. She recently completed her doctoral degree at the University of Maryland School of Nursing, where she received a dissertation award from the Health Care Financing Administration for her research focusing on the impact of postprostectomy urinary incontinence on psychosocial adjustment to illness and quality of life. Her current research, funded by the Department of Defense, will continue to explore postprostatectomy incontinence with a focus on ethnic identity. During the past several decades Dr. Powel has been a frequent contributor to the oncology nursing literature focusing on issues of quality of life and the management of treatment-related symptoms in patients with cancer.
