In this monograph, the authors summarize their findings in complex neuroimaging work (cranio-, spondylo-, myelo- and angiography as well as CT and MR imaging of the brain and spine) during their longstanding experience at the N. Burdenko Neurosurgical Institute in Moscow. The book begins with a review of modern neuroimaging techniques: CT and MR angiography, perfusion and diffusion imaging, tractography, spectroscopy and functional MR imaging.
The problems and various other aspects of diagnosis of intra- and extra-axial brain tumors (more than 30,000 verified cases) as well as of cerebrovascular, infectious, demyelinating, degenerative and traumatic brain and spine lesions are discussed. The volume is well illustrated with angiographic, CT and MR images of complex diagnostic studies. The numerous images represent a "visual text," which can be used as an atlas by practical clinicians.

The book contains 15 chapters. The first chapter outlines the stages of formation and development of neurovisualisation technology, with a historical background, and gives information necessary for understanding the novel diagnostic methods that help to visualise pathology in the context of anatomical and pathophysiological changes, even on the molecular level. New diagnostic technology makes it possible to obtain quantitative estimation of changes in disease progression, and it plays an important role in making early diagnoses within routine protocols of investigations. The chapter is aimed at readers who are familiar with the basic principles of CT and MR imaging, which is why in it we consider only the main principles and practical aspects of the newest quantitative CT and MRI diagnostic technologies, including diffusion MRI and diffusion tensor imaging, CT perfusion and MR perfusion, functional MRI, proton spectroscopy, and CT angiography and MR angiography.
In other chapters, we discuss the diagnostic principles of the main categories of diseases of, and damages to, the CNS. Most of this volume is devoted to the diagnoses of cerebrovascular disease, intracerebral and extracranial tumours and tumour- like processes, inflammatory, demyelinative, and neurodegenerative disease, and traumatic brain and spinal damage.
Chapters dedicated to the diagnoses of brain tumours are based on verified clinical cases (more than 30,000) using the complex approach including MRI, MRS, CT and MR perfusions, and diffusion-weighted imaging. Proton MR spectroscopy information is given in single- and multivoxel-regime formats Each clinical chapter contains the most important information on CNS pathological changes, basic markers for specific diagnose of the diseases, and unique images of rare pathologies.

Despite technical advances using MR and the emergence of PET imaging, CT continues to be a frequently used imaging modality that has not stood still while others have thrived. Advances in CT have continued apace with other modalities which have taken CT into new areas of imaging that just a few short years ago it could not approach. With the advent of 64-slice CT, a greater number of images can be taken in a much quicker period of time. The use of CT perfusion in the brain and abdomen are reviewed in this issue. Cutting edge techniques such as 4D CT and dual energy CT will be discussed, as well as CT applications for imaging of the coronary arteries and cardiac valves.
Despite the availability of MDCT scanners for a decade, the technology remains in flux. In the recent past, 64-MDCT scanners were considered the top of the line equipments; however, several newinnovations inCTare nowavailable oremerging to embrace the patient care by using more detector rows (128-320 detector rows), new detector materials for higher sensitivity andultrafast energyswitching (Gemstone technology), two X-ray sources (dual-source CT), detectors with 2 layers to permit spectral analysis of the X-ray beam, and systems with flat-panel detectors (dynamic volume CT).
These exciting new opportunities available with MDCT would not have been feasible without focus and dedication of countless passionate physicians, scientists and engineers. We have a distinct privilege to introduce this issue of Radiologic Clinics of North America that focuses on these new or emerging technologies in MDCT. We are honoured that our panel of distinguished MDCT scientists have contributed articles to this issue in their respective area of expertise. Preface:
Since the first edition of Computed Tomography of the Coronary Arteries was published in 2005, knowledge about CT technology has significantly increased, which prompted us to provide an updated version of the book. Non-invasive coronary imaging using multislice CT has rapidly evolved as a diagnostic modality to detect or exclude the presence of significant coronary artery disease. In addition, multislice CT is able to provide important information about non-obstructive plaques and their tissue composition, including whether they are noncalcified, calcified, or mixed coronary plaques. The technological improvements of CT scanners have been impressive and 64-slice CT has largely superseded four- and 16-slice CT scanners, while prototype 256- or 320-slice CT scanners have been introduced very recently. Many reports have been published about the diagnostic performance of CT coronary angiography to detect significant coronary stenosis compared with invasive coronary angiography. These reports unanimously agree that CT coronary angiography can reliably exclude the presence of significant coronary artery disease but that the accurate detection of coronary obstructions, i.e. extent, location, and distribution, is still somewhat limited, and requires improvement in spatial and temporal resolution.
Cardiac CT has matured significantly and has attracted many radiologists and cardiologists to begin a cardiac CT program in their departments. We also believe that cardiac CT has a bright future and may become the most important non-invasive modality for the visualization of the coronary arteries.
The CT technique, although relatively straight forward, requires thorough understanding of the basic principles for accurate interpretation and clinical application of CT coronary images.
Many cardiologists and radiologists are not familiar with cardiac CT, and we believe that our book provides easy and understandable information about cardiac CT. The book is the result of close collaboration between both cardiologists and radiologists, and the contents are a reflection of the specific insights of each discipline. We clearly believe that for optimal interpretation and implementation of cardiac CT teamwork between radiologists and cardiologists is essential.
We hope that the second edition will again serve its goal as a concise, quick reference for understanding and interpreting CT coronary images.
