Surgery offers a highly efficient treatment option for patients with pharmacoresistant epilepsies. While most surgical procedures aim at curative treatment, others have been designed for palliative purposes. Although standardized procedures are available, surgical techniques actually applied vary greatly. However, to date no comprehensive overview on different methods is available. The book by Zentner and Seeger compensates for these shortcomings. It comprises the wide spectrum of current procedures and provides a thorough description of surgical steps. Moreover, detailed information as to results and complications of different procedures is given. Thus, this book not only contains practical instructions for neurosurgeons, but also enables neurologists and epileptologists to understand possibilities and limitations of epilepsy surgery and facilitates adequate consultation of patients and relatives.

In the majority of patients with epilepsy, seizures can be well controlled with appropriate medication. However, current estimates indicate that 20 - 30% of patients with epilepsy are refractory to all forms of medical therapy.(1) These medically intractable patients are candidates for surgical treatment in an attempt to achieve better seizure control. Another group of patients who might benefit are those whose seizures may be relatively well controlled but who have certain characteristic presentations or lesions that strongly suggest surgical intervention might be curative. Overall, the single most important determinant of a successful surgical outcome is patient selection. This requires detailed presurgical evaluation to characterize seizure type, frequency, site of onset, psychosocial functioning and degree of disability in order to select the most appropriate treatment from a variety of surgical options. This type of evaluation is best carried out at a multi-disciplinary center experienced in the investigation and treatment of epilepsy.

This publication is primarily directed to the amplification of a new treatment modality that addresses only a fraction of the problem. It is, however, a quantum leap in the evolution of fixation with compression hip screws which are still the gold standard for surgical stabilization of pertrochanteric hip fractures.

The Dynamic Martin Screw (DMS) addresses the issue of adjustability of the fixation angle with appropriate mechanical strength characteristics that were lacking in its historical predecessors.

It exhibits the unique ability to allow the surgeon to improve the reduction to a more advantageous valgus angle even after it is rigidly attached to the femur. This provides a more stable construct that will allow for a more successful recovery of the patient to their previous ambulatory life.

In addition, the device is ideally suited for a new challenge in hip fractures, the MRI fracture. These are undisplaced fractures diagnosed almost entirely by magnetic resonance imaging. Since they are, by definition, undisplaced, the infinite adjustment of the femur neck angle of this device is the perfect solution.

The variable angle system has been adapted for paediatric use. While useful for fracture fixation, it is an excellent option for fixation in the varus femoral osteotomy for dysplastic hips and subluxation in children with cerebral palsy.

Further on the supracondylar version is very valuable for distal femur fractures. The supracondylar version with cable extraordinarily is suited for the devastating periprosthetic femoral fracture in the total hip patient. This situation, by the way, is a new epidemic!
